Grant supports PHASE Plus program

Tri-City Health Center (TCHC) was awarded a $15,000 grant by Kaiser Permanente Community Benefit Program in southern Alameda County for the TCHC PHASE Plus program.
Preventing Heart Attacks and Strokes Everyday (PHASE) is Kaiser Permanente's population-based approach to ensure comprehensive secondary prevention of atherosclerosis in patients with chronic diseases. The TCHC PHASE Plus program is an extension of Kaiser Permanente's PHASE approach, integrating chronic care management model to improve overall wellness for patients diagnosed with hypertension and diabetes.
The TCHC PHASE Plus program will promote comprehensive secondary prevention of atherosclerosis to 100 hypertensive patients with a likely co-morbidity of diabetes by providing proven preventive medications, achieving control of their key risk factors, integrated health consultations, and wellness education. The TCHC program is designed to teach patients to better manage their disease in their everyday lives, while providing them with vital medical care. This integrated chronic case management along with behavioral health process will help patients gain knowledge in order to cope with and control their disease and improve future health outcomes.
